Who Attended a POLST Discussion?
Deadline for Section C is Quickly Approaching
 
After June 30th, any new POLST forms received by the Oregon POLST Registry not indicating who participated in a POLST discussion and leaving Section C blank will not be accepted by the Oregon POLST Registry and will be returned to the sender as Not Registry Ready (NRR).
Documenting who participated in the discussion helps to reinforce the patient’s right to a support person(s) for these important conversations, while strengthening the signing health care professional's attestation that core POLST principles were preserved.
This new requirement will only affect paper POLST forms, as the mandatory hard-stop already exists within all Oregon ePOLST systems.

Help Disseminate the First 9 Translated
Versions of the Patient Factsheet
Now that we have completed the initial nine languages of the POLST Patient Factsheet, we are looking to get the word out to anyone who serves these multicultural communities.

The project to translate the English version of the POLST factsheet for patients and other laypersons into clear language versions began in mid-2021. Each version has been reviewed for cultural appropriateness of text and images by beta-testers from each population’s community. We worked with Lorena Sprager and her colleagues to engage Community Health Workers and others within each of these non-English speaking communities.

Spanish, Chinese (Simplified & Traditional), Korean, Vietnamese, Russian, Ukrainian, and Arabic versions are complete and available on the Oregon POLST Program's website on the Patients & Family page located under A Factsheet for Someone New to POLST. The Japanese translation is currently in production/beta-testing process.

2023 POLST Changes Outlined for Health Care Professionals in an Upcoming Educational Video
A 5-minute educational video specifically tailored to health care professionals is in production. It will outline changes in the newest version of the Oregon POLST form, as well as review core POLST principles. Once completed, the video will be posted to our YouTube Channel.

Watch for announcements prior to its release later this month.
