Oregon POLST Form to be revised for 2018
SB 856 (Allows Naturopathic Physicians to sign POLST) goes into effect on January 1, 2018. In response to this statutory requirement, the POLST form will be revised. All prior year form versions will continue to be honored. Like all previous versions, the 2018 POLST form will be a result of changes and updates suggested by POLST stakeholders, quality data from the Oregon POLST Registry, and the addition of effective elements from other state POLST forms. The revised (version 12) of the Oregon POLST form and a summary of the changes will be shared at the end of November.
Oregon POLST Coalition launches Quality Committee
The Oregon POLST Form Committee has been renamed the Quality Improvement Committee, which will better reflect the Oregon POLST Coalition's greater focus on quality improvement. This committee will be chaired by the Nurse Practitioners of Oregon representative, Pam Hiransomboon-Vogel, DNP, APRN, FNP-BC, ACHPN, and the first task will be the revision of the Oregon POLST form.
Oregon POLST separates from the National POLST Paradigm
In July 2017, the Oregon POLST Coalition reaffirmed its foundational values of not accepting support from the health care industry. Actions include: 1) clarifying the Center for Ethics policy on conflicts of interest (a policy that the Coalition honors as the Center serves as its administrative home) and 2) withdrawing membership from the National POLST Paradigm Task Force (NPPTF) because of its 2017 policy change to accept health care industry funding.

"Understanding POLST" reaches 10,000 YouTube views
The Oregon POLST Program produced the "Understanding POLST" video as means to provide a clear, detailed overview of the Oregon POLST Program and form. The video uses language that individuals without a health care background can understand. All Oregon POLST videos can be found on the Oregon POLST YouTube channel or at www.oregonpolst.org.
2016 Oregon POLST Registry Annual Report
A message from Dana Zive, Director, Oregon POLST Registry
The Oregon POLST Registry team is very pleased to present its annual report for 2016. Once again, we saw our highest volume ever in POLST form submission, calls to the emergency communication center, as well as the POLST business office, while maintaining research and educational efforts, new approaches to changing technology, and the increasing use of electronic POLST form completion systems.

We are very grateful for our State partners, the Oregon POLST office and Coalition, and all those who work to support the availability of POLST orders when they are most needed.

Please let us know if you have any comments or questions, and please visit us at www.orpolstregistry.org for more information.
