Oregon becomes the first state to remove feeding tubes from POLST forms
On January 2, 2019, Oregon became the first state to remove artificially administered nutrition from their POLST form. A recently published article in the Journal of the American Geriatrics Society discusses the reasoning for this significant revision and provides a recommendation for other state POLST programs to consider. Tips for Submitting to the Oregon POLST Registry
Please reassure patients who have completed an earlier version of the POLST form that their current form remains valid.

To submit to the Registry the following legible information must be completed:

  • Patient's full name
  • Date of birth
  • M.D. / D.O. / N.P. / P.A. / N.D. signature (with legible printed name)
  • Date signed by the M.D. / D.O. / N.P. / P.A. / N.D.

The front and back of POLST forms should be faxed (with a cover sheet) or mailed to the Registry (unless the "Opt Out" box is checked).
