A New POLST Form for 2023
 
Oregon's 14th version of the POLST form is available now!
Major highlights include:
  • Pre-existing POLST forms remain valid so long as the patient’s wishes have not changed.  A new POLST using the revised 2023 form does not need to be completed.
  • If “Attempt Resuscitation/CPR” is selected, then Full Treatment must be chosen in Section B. This change was made to reduce confusion for emergency personnel.
  • “Limited Treatment” has been changed to “Selective Treatment” in Section B. This change improves our alignment with neighboring POLST programs.
  • Documenting who participated in the discussion is now required in Section C. Requiring documentation of those individuals who participated in the discussion reinforces the patient’s right to support person(s) for these important conversations.

has been created as a new educational tool.

Independent and assisted living facility personnel and other
non-medical professionals like estate planning attorneys who
offer POLST education may use this sample form
to help educate their residents and clients about POLST.

Oregon Dept of Human Services' POLST Alert for Long-Term Care Facilities & Group Homes
New POLST guidance has been published by Oregon DHS and can be found at their website and linked here as the ODDS TransmittalAPD Provider Alert.

Chinese Versions of Patient Factsheets Further Expand our DEI Efforts for POLST Accessibility
Traditional and Simplified Chinese versions of our lay POLST factsheet have been added to our resources library for patients, their families and caregivers.

These additions join the
existing English, Spanish and Vietnamese versions. By increasing our culturally specific educational materials, we work to empower more Oregonians to decide if POLST is right for them.

A Korean version has been field tested recently with our partner community health workers and will be offered soon! Look for Russian and Ukrainian versions in the near future at: https://oregonpolst.org/resources/#factsheets-articles.

Christine Mullowney, MD Elected as
New Chair of O
regon's POLST Coalition

Christine Mullowney, MD has been voted as the Oregon POLST Coalition's new Chair after serving as its Co-chair over the past year.

Christine brings more than a decade of experience as an OHSU primary care physician along with her expertise as a quality improvement leader to this role.
 
Susan Tolle will remain on the Coalition as its Co-chair, while continuing to provide background information related to POLST and its decades-long history.

Susan, who has served on the Coalition since its inception, will take advantage of this exchange in roles to direct some of her time and energy into supporting the launch of a new major initiative related to compassionate communication at OHSU's Center for Ethics in Health Care.

Dr. Mullowney's position as Chair was formally adopted at the Coalition's quarterly meeting on February 2nd.
The mission of the Oregon POLST® Coalition is to improve the quality of life for Oregonians nearing the end of life by providing an evidence-based, patient-centered, voluntary process that elicits, records and honors the treatment goals of those with advanced illness and frailty in a compassionate manner that is respectful of the inherent dignity of the individual.
